I like to think it works something like the bombardier beetle's chemical spray, where two chemicals are mixed together in a specialized series of chambers and then ejected at high speed. I think this is the case because dragon fire seems to behave more like napalm or another flammable liquid rather than an expelled gas, though this kind of depends on the depiction. This might also explain why some art of dragons depict them as having very large chests, as this might be where the aforementioned chambers are housed.
