Well, essentially because the piston is still guided by the cylinder and can't be pulled out unless you take out the whole crank assembly.
The term "hit and miss" has more to do with the combustion cycles, where most hit and miss engines, once overspeeding, will leave open their exhaust valve to prevent the build up of pressure, and thus combustion, to "miss" the stroke in order not to overspeed.
